Section 3. Physical Participation Acknowledgement

FAB Training courses may involve practical activities such as:


• Demonstration and practice of hands on skills, including CPR where relevant

• Moving into seated, kneeling, or floor based positions

• Practical scenarios, role play, or working with other participants

• Using training equipment or manikins


You are encouraged to work within your own comfort and physical limits at all times. You may opt out of any activity, or request an adaptation, if you feel unable to participate safely.

Section 4. Declaration

I confirm that the information provided is accurate to the best of my knowledge.


I understand that:

• FAB Training will use this information solely to support safe delivery of the course

• Participation in practical activities is voluntary and can be adapted

• I am responsible for informing the trainer if I feel unwell or unable to participate at any point during the session

Information will be:

• Accessed only by authorised staff involved in course delivery

• Used solely for health, safety, and training delivery purposes

• Retained securely for up to 3 years from the course date in line with standard health and safety and legal practices, after which it will be securely deleted.
